Participate in functional patient care conferences and coordinate with physicians, social workers, and other members of the clinic staff to care for patients with multiple chronic diagnoses and assess social determinants of health. Work with the providers and clinic staff to identify patients at high risk. Perform assessment of health conditions and medication reconciliation. Provide education to patients on how to manage chronic conditions. Coordinate with providers to establish or update care plans. Perform accurate and timely documentation in patients’ electronic medical records. Perform clinical tasks within scope of practice.
